Can somebody please explain this game to me?

Is it each side has half the town after them with each side having their own informed Mafia group that wins with their group and a cop/ doctor that protects their own group but kills the other group? And what's the deal with the metrosexual? Like even without the theming I feel like this could never have a game run this way today.

Also the 2020 instalment was, well tame, and I actually understood what was happening there
