How about this?
If the mafia outnumbers town and the survivor is still alive, the goons lose (i.e. - the survivor wins alone)
The mafia can't NK one of its own members.
Mafia have to kill every night.
I don't know, is this a stupid idea? I thought it would be interesting to have kind of a game-within-a-game (the mafia has to determine who to bus), but please tell me if no one would actually want to play this game.
What is a Mafia Survivor? Is it a survivor that appears to be a goon for the goons?
But I think the set-up looks great

Misslynch D1 and town NK by both Werewolf and SK and werewolves has won D2. Need more town roles.
What if Werewolves only win if they're the only faction rather than by endgaming? That way they would have to lynch the SK.
Werewolfs would still control 50% of the votes if there is a mislynch D1 and just NK on town players. If either a townie/SK votes a non-werewolf, they could just wagon that vote and still win that game.

Elaboration on "This Rupert Guy"
This Rupert Guy is a mail carrier. While he is alive, each and every single person can send one letter to another person during the night. If he dies (by any means), however, the mail ability is disabled for everyone. He's more of a lurker, and should not post as much as other players. I'd implement post restrictions for him. Furthermore, he has the ability to watch someone at night, regardless if anyone sends any mail. He is a survivor. (For an additional challenge, make it so that if This Rupert Guy is killed, nobody can nightchat.)
This Rupert Guy is from some game called "Earl Grey and This Rupert Guy". Go play it.
I like most of this, but when This Rupert Guy dies, is it really worth for everyone to not being able to nightchat. Mailing could be a really swingy mechanic.

IDK know about the mafia doctor. Could give scum a free extra night-kill if one town doctor announces who they're gonna protect. Personally I like the version w/o the mafia doctor beter

The game starts on Night 0. possibly people die.
On Day 1, it is announced who "died" but not their alignments. If they would like to talk during the day they can, but cannot vote, and do not count towards voting majority. Someone is lynched.
On Night 1, Dr. Frankie can pick one of the N0 targets or the D1 lynch to come back to life the next day. Frankie can also choose not to save anyone. Also on Night 1, there are more "deaths".
On Day 2, it is revealed who Frankie saved. That person is back in the game, alive and voting. The other two (or all three, if no one is saved) have their alignment revealed, and are dead for good. Repeat.
I like the concept of this. But I think that the game needs to be a 9:2:2 game in order to work out

Simultaneously. But I can also imagine giving the extra power just to VTs.
Not a good idea. The game becomes way to swingy then.
